Summary/Objective:

Responsible for assessing client’s needs and capabilities, providing supported living services, skills enhancement, medication monitoring, and supportive counseling to facilitate progress towards rehabilitation plan goals. Establishes mental and medical health referrals and utilizes available resources in the community.

Duties and Responsibility:

  • 1. Face-to-face contact with clients as planned.
  • 2. Complete visit contact notes and any other applicable contact notes for each client.
  • 3. Provide supportive services that assist clients in maintaining safe and healthy lifestyle and living environment.
  • 4. Accompany, and/or assist client with getting to appointment as necessary while engaging in skill-building activities related to client’s IRP Goals.
  • 5. Participation in individual IRP goal development to facilitate client working towards and accomplishing their goals with supervisor.
  • 6. Serve as liaison between clients and mental health professionals, client’s family, therapist, community agencies, housing providers, and others involved in the process of problem resolution and on-going case management.
  • 7. Perform crisis intervention and assistance as necessary.
  • 8. Monitor client’s clinical/physical health and medications

Direct Care Worker Expectations:

  • Meet with clients weekly for a total of 6 times per month for adults and enter contact note within 48 hours of service.
  • Collaborate with therapist about client’s progress monthly to ensure compliance.
  • Attend and participate in trainings as offered
  • Attend weekly meetings with supervisor
  • Research and provide resources to your clients
  • Help to develop and maintain a Resource Manual
  • Notify supervisor of all crisis situations to help with decision making
  • Submit any outstanding notes and client monthly contact log
  • Discuss all intended discharges with supervisor before beginning the process.
  • Complete all discharges and submit paperwork to supervisor

Supervisory Responsibility

The program manager is responsible for oversight of all office staff.

Work Environment

This position works in an outpatient mental care facility. There will be frequent interaction with persons who are mentally ill, disabled, elderly and/or emotionally upset.

Physical Demands

Mobility and ability to sit, bend, reach and lift 10 lbs. Prefers CPR/First Aid Certified.

Position Type and Expected Hours of Work

This is a part-time position that will grow to a full-time position; expected work hours are going to be 10-40 hours per week, Monday through Friday, and dependent upon caseload and initiative. Must see each client 6 times per month.

Travel

Travel is required.

  • High school diploma or equivalency
